I, of legal age, acting on my behalf and as the legal guardian of my child participant, hereby execute this release, forever absolving and exonerating Hoops4Life, Inc., along with its officers, directors, agents, employees, and parent chaperones, from any and all liability, claims, or demands, including legal defense costs, of any nature whatsoever, whether incurred or experienced by myself or my child participant during their involvement in the Hoops4Life Program ("Program"). This encompasses activities such as field trips and related endeavors.

Moreover, I hereby assume, on my behalf and on behalf of my child participant, all risks associated with personal injury, illness, fatality, property damage, and expenses incurred in connection with the Program. I grant chaperones the authority to provide necessary transportation, food, and accommodation for my child during these excursions.

As the parent(s) or legal guardian(s) of the participant, I hereby authorize full participation in the Program and provide consent for any required medical treatment, including but not limited to emergency surgery or medical care, assuming responsibility for any resultant medical expenses, if applicable. Furthermore, in the event that the participant must return home due to medical reasons, disciplinary actions, or any other cause, I accept all associated transportation expenses.


I hereby grant Hoops4Life, Inc. the permission to publish, in print, electronic, or video formats, any likeness, image, or audio recordings of my child. I relinquish all claims related to copyright ownership and publication, including any claims for compensation associated with the use of these materials. Images, videos, and audio recordings of my child captured or recorded during their participation in Hoops4Life, Inc. may be utilized in print, broadcast, and various advertising formats, such as brochures, newsletters, and other publications, as well as on the Hoops4Life, Inc. website, in audiovisual presentations, and in other promotional activities aimed at raising awareness about the organization and informing the public. The organization is authorized to employ these photos, videos, or other visual or auditory recordings without the obligation of paying fees, royalties, or any other form of compensation.


It is advisable to secure a release when photographing or videotaping a minor (under 18 years of age). Parent or guardian signatures are mandatory, and the signatures of minors themselves are not deemed sufficient. In cases where images are to be published, Hoops4Life, Inc. will exercise due care to limit the disclosure of identifying information and will abstain from using specific street or mailing addresses, email addresses, or phone numbers.

Signed release forms are not required for subjects in public areas, such as fairgrounds or parks. However, when photographing or videotaping within private or public schools or youth camps, it is essential to obtain the organization's permission and have signed release forms from a parent or guardian for each child involved. The responsibility for acquiring these signed release forms and maintaining records lies with the photographer or videographer.
